To make the onboarding process as smooth as possible, please plan to spend approximately 15 to 20 minutes completing this form.

Before you begin, we recommend having any required documents readily available, such as professional licenses, certifications, insurance information, fingerprint clearance cards, or other requested paperwork.

What You May Need

Depending on your role, you may be asked to provide some or all of the following:

• Government-issued photo ID

• Resume

• Professional license (if applicable)

• Professional liability insurance

• CPR certification

• Article 9 certification (if applicable)

• Fingerprint Clearance Card

• Criminal History Self-Disclosure Form

• Direct Service Position documentation (if applicable)

• NPI or AHCCCS information (if applicable)

• Signed employment or onboarding documents

What Happens After You Submit?

Once we receive your onboarding:

✅ Step 1: Our team will review your information and confirm that all required documents have been received.

✅ Step 2: Scott (or a member of our administrative team) will reach out to discuss your availability, preferred service areas, and scheduling preferences.

✅ Step 3: Once everything is confirmed, we'll begin matching you with children and families that align with your schedule, experience, and location preferences.

✅ Step 4: We'll stay in communication as your caseload grows and continue supporting you throughout your time with Pure Hearts Therapy.
