Pediatric Occupational Therapist (OTR/L) / COTA – Home Health

Pure Hearts Therapy is hiring Pediatric Occupational Therapists (OTR/L) and Certified Occupational Therapy Assistants (COTA) to provide home-based occupational therapy services to children with developmental disabilities across Phoenix and surrounding Arizona communities.

This is a flexible, relationship-driven pediatric home health role for independent contractor occupational therapy practitioners who value autonomy, meaningful sessions, and family-centered care.

Position Details

  • Position: Pediatric Occupational Therapist (OTR/L)

  • Location: Phoenix, AZ, and surrounding communities

  • Setting: Home-based pediatric therapy

  • Schedule: Flexible, based on your availability

  • Employment Type: Independent Contractor (1099)

What You’ll Do

  • Provide individualized pediatric occupational therapy services

  • Address sensory processing, fine motor skills, gross motor skills, self-care, and regulation

  • Collaborate closely with caregivers to support carryover into daily routines

  • Complete evaluations (OT), care plans (OT), and progress reports in compliance with requirements

  • Communicate with families, support coordinators, and interdisciplinary team members

Requirements

  • Active Arizona Occupational Therapist (OTR/L) or Occupational Therapy Assistant (COTA) license

  • Level 1 Fingerprint Clearance Card

  • Article 9 certification

  • CPR / First Aid certification

  • Professional liability insurance

  • AZ Centralized Background Check (CBC)

  • Reliable transportation for home visits

Pediatric experience preferred. New graduates welcome with pediatric fieldwork experience.
